Self Service Price Estimator

To complete your own estimate for some of our most frequent services please click on Price Estimate. If you need assistance or want an estimate for a service not listed in the self estimator tool please email the following information to estimates@fulhealth.org or call 419-330-3308.

You will need the following information in order to complete a price estimate:

• Patient Name
• Patient Date of Birth
• Patient Gender
• Insurance name
• Insurance ID number (and group number)
• CPT code or complete description of service to be estimated  (or send copy of your provider’s order)  CT and MRI’s must specify if contrast is ordered
• Diagnosis for service (or send copy of your provider’s order)
• Please specify how you would like us to provide you your estimate (Secure email, USPS mail, or phone call and provide details to fulfill)
