Frequently Asked Questions

The short answer is “yes, most of the time”, but as with any other medical procedure it will largely be dependent on your insurance coverage. The discharge planning staff at FCHC does an excellent job of recognizing the limitations and communicating that to the Providers and the patients we serve.

This is all driven by the Doctor / Provider. If you are admitted for an illness or a surgery, your Provider will determine if that condition requires the needs of the Rehab Team. Often, if there are questions or concerns about your ability to safely return home, Rehab will be ordered to assess that. If you feel that you may benefit from Rehab during your stay, feel free to discuss that with your Doctor and Nurse.

This will depend on your Therapy evaluation, and the recommendations of the Therapist(s). It could be 1-2 sessions per day, per discipline, again depending on your specific noted needs.

The Therapy Team and Discharge Planning offices work together and will keep you involved in the potential decisions for the next level of care. That may be asking for your top choices on Rehab Facilities to consider, if needed.  The decision comes down to insurance coverage, and the availability of the Facility.
Yes, this is something that may be a sought-out option after being discharged home depending on the recommendations of the Doctor/Provider and Therapy teams.
