The Health Information Department at Fulton County Health Center helps you obtain medical records for care received at FCHC. We can also help you get set up on the Patient Portal, where you can securely view records anytime and download them for future needs.
How to Obtain Your Medical Records
Complete the FCHC Authorization to Release Patient Information form. This authorizes us to release records to you and/or a recipient you name. Download the Release of Information form (PDF)
Fax your completed form to the Health Information Department at 419-337-7324 or 419-330-2616, or return it in person to the department on the first floor of the hospital.
There may be a charge for medical record copies.
Bring your photo ID when you pick up your records.
Records may be mailed to your home or to another authorized recipient. Prepayment may be required when applicable.
The FCHC Patient Portal also offers limited information — lab results, testing results, and more — that you can view online. Go to the Patient Portal
