WAUSEON, Ohio – Fulton County Health Center is pleased to welcome two experienced healthcare providers to its growing Primary Care team. Dr. Jana Bourn, MD, is now accepting both new and existing patients at FCHC Primary Care in Wauseon, and Stefanie Johnson, NP, has joined FCHC Primary Care in Delta.
The addition of these providers reflects FCHC’s ongoing commitment to expanding access to high-quality, compassionate primary care throughout Fulton County and the surrounding communities.
Dr. Jana Bourn, MD – Wauseon
A lifelong Fulton County resident, Dr. Jana Bourn is honored to care for the community she has always called home. Born and raised in Wauseon, she now lives there with her husband, Rob, and their five children.
As a board-certified primary care physician, Dr. Bourn provides comprehensive care for patients of all ages—from pediatrics through older adulthood—helping individuals and families stay healthy through every stage of life. In addition to caring for patients, she has served as Medical Director for the Fulton County Health Department for more than 10 years, demonstrating her longstanding commitment to improving the health of our local communities.
When she’s not caring for patients, Dr. Bourn enjoys spending time outdoors hiking, boating, and biking with her family.

Stefanie Johnson, NP – Delta
Originally from Deshler, Stefanie Johnson has been serving patients as a Nurse Practitioner since 2016. She brings valuable experience in acute stroke and neurology care, as well as urgent care medicine.
Stefanie is passionate about caring for patients in rural communities and is excited to provide quality, personalized healthcare close to home.
“I’m excited to be here in Delta and enjoy caring for patients in a small-town, rural community,” said Johnson. “Building relationships and providing personalized care close to home is what I love most.”
Her patient-centered approach focuses on preventive care, managing chronic conditions, treating acute illnesses, and partnering with patients to achieve their long-term health goals.
